Please complete a Referral Form (doc) and you can fax it to our office 310.998.2274.
You may also phone our office at 310.998.CARE (2273) for more information or to speak directly with Dr. Waldman or with Dr. Kramer.

Prior to your client's appointment at our clinic, please fax the patient's recent medical records including surgery reports and laboratory tests. Please have your client bring their most recent imaging studies as well. If, during the course of our treatment, you are to perform additional imaging studies, please also send those with the client for our viewing information. In addition, please alert us to any existing medical or surgical conditions, including current medications.

At your client’s initial appointment, we will structure an in-clinic therapy plan for the patient. Additionally, a home exercise plan will be issued which will be updated as the patient progresses through therapy. A written summary of their visit will be mailed or faxed to you so that you will be able to keep your patient's medical record complete.

We welcome your telephone calls, faxes and e-mails. If you would like to consult with Dr. Waldman on a particular case or have questions regarding a patient that is currently undergoing treatment at our facility please contact us.
